What is Postoperative Pain?
Postoperative pain is a complex physiological and psychological response to surgery, generally resulting from tissue damage, inflammation, and the body's stress response. Effective management of postoperative pain is important for recovery because uncontrolled pain can interfere with healing, increase the risk of complications, delay recovery, and potentially contribute to chronic pain.
Acute postoperative pain generally occurs immediately following surgery and may last for days to weeks because of tissue injury and inflammation. Chronic post-surgical pain persists for more than three months and may affect 10-50% of patients, depending on the type of surgery and individual risk factors. Neuropathic pain may also occur when surgery causes nerve damage, resulting in burning, tingling, or shooting sensations.
Surgical injury to tissues and nerves can activate inflammatory pathways and alter pain processing. Peripheral nerve injury may increase sodium-channel expression in sensitized nerves, leading to spontaneous activity and increased glutamate release. Glutamate can act on receptors such as NMDA and AMPA, contributing to central sensitization and amplification of pain signals within the spinal cord.
The intensity of postoperative pain varies according to the type of surgery, with procedures involving substantial tissue trauma, including orthopedic, thoracic, and abdominal surgeries, generally associated with more severe pain. Age, sex, genetics, previous pain experiences, anxiety, depression, and expectations regarding postoperative pain can also influence pain perception.
Management commonly involves a step-wise and balanced approach. Multimodal analgesia combines two or more analgesic agents with different mechanisms of action to achieve pain relief while limiting adverse effects. Common intravenous opioid analgesics include morphine, hydromorphone, and fentanyl.
Emerging Postoperative Pain Drug Profiles
PF614: Ensysce Biosciences
PF614 is an oxycodone-based pain medication that has been chemically modified to remain inactive until swallowed, where digestive enzymes activate and release oxycodone. The therapy is designed to provide pain relief for severe pain requiring opioid treatment.
PF614 uses Ensysce's TAAPTM technology, which chemically modifies the opioid to provide controlled activation after oral administration. According to the DelveInsight report, PF614 is designed to provide approximately 12 hours of pain relief per dose and can be taken with or without food. PF614 is currently in Phase III development for Postoperative Pain.
ATX101: Allay Therapeutics
ATX101 is an investigational formulation combining bupivacaine, an established intracellular sodium ion channel blocker, with a biopolymer designed to provide prolonged postoperative analgesia. The formulation is intended to provide weeks of pain relief following total knee arthroplasty.
ATX101 is placed at the end of standard surgery and is designed to provide ultra-sustained local analgesia before eventually dissolving into water and carbon dioxide. The approach is intended to reduce reliance on complex combinations of shorter-acting analgesic products used following surgery. ATX101 is currently in Phase II clinical development for Postoperative Pain.
LC-400: LipoCure
LC-400 is an ultra-long-acting local anesthetic (ULALA) developed by LipoCure to provide extended postoperative pain relief following a single administration. It consists of a multi-vesicular liposome hydrogel formulation containing bupivacaine, enabling controlled and prolonged drug release.
By maintaining sustained bupivacaine levels at the target site, LC-400 is designed to reduce the need for additional analgesics such as NSAIDs or opioids and potentially reduce their associated side effects. Phase I clinical studies have generated initial data supporting continued development, with the drug advancing toward Phase II trials.
GB-6002: G2GBIO
GB-6002 is an extended-release injectable formulation utilizing ropivacaine, designed for direct administration at the surgical site. The localized approach is intended to reduce systemic drug exposure while providing prolonged postoperative pain relief.
The therapy is designed to provide pain relief for more than three days following a single administration. According to the DelveInsight report, G2GBIO has completed a clinical pharmacology study demonstrating favorable pharmacokinetic results, safety, and tolerability. GB-6002 is currently in Phase I clinical development for Postoperative Pain.
