Empowering San Francisco: SFPHF's 2024 Health Equity Grants

San Francisco, CA - October 18, 2023 - The San Francisco Public Health Foundation (SFPHF) is excited to announce the release of the 2024 Healthy Communities SUPPORT Grants Request for Proposals aimed at promoting health equity and addressing diet-sensitive chronic diseases. These grants are designed to empower non-profit agencies in San Francisco to implement initiatives focused on healthy eating and active living for Priority Populations.

The 2024 SUPPORT Grants consist of two main categories:

Capacity Building Grants: SFPHF plans to award a minimum of five grants, each with a maximum value of $60,000, to support non-profit agencies in strengthening their capacity. These grants are expected to last approximately five months and will provide one-time funding to purchase essential resources such as equipment, data systems, computers, software, curriculum, consultants, or other tools and services that will enhance their ability to deliver programs promoting healthy eating and active living to Priority Populations.

Event Sponsorship Grants: In addition to Capacity Building Grants, the RFP includes a limited number of Event Sponsorship Grants ranging from $5,000 to $10,000. These grants are intended to support events that align with the promotion of healthy eating and active living, with a focus on reaching Priority Populations in San Francisco.

To be eligible to apply for these grants, organizations must meet the following criteria:

Be non-profit agencies and organizations actively delivering diet-sensitive chronic disease interventions to San Francisco's Priority Populations.

Demonstrate a track record of effectively reaching Priority Populations, particularly those most impacted by sugary drink consumption.

Qualify as non-profit community, faith, or neighborhood-based organizations (CBO/FBO/NBO).

Agencies without non-profit status may apply with a 501(c)3 nonprofit agency serving as a fiscal sponsor for their project.

Key Dates for the 2024 Healthy Communities SUPPORT Grants:

Get the RFP. The RFP will be available from October 16, 2023, providing interested organizations with a clear understanding of the application process and requirements.

Submission deadline. All proposals must be submitted by 12:00 noon on November 16, 2023. This deadline marks the closure of the application period, and submissions received after this date will not be considered.

Award Notifications. Award notifications are to be sent out by mid-January 2024.

The primary goal of the SFPHF Healthy Communities SUPPORT Grants is to promote health equity by providing short-term funding with lasting effects beyond the grant period. The foundation understands the need for robust capacity building and event sponsorship to advance health initiatives in San Francisco, particularly for Priority Populations who face the greatest challenges in terms of diet-sensitive chronic diseases.
