Efficiency of a Membrane Oxygenator

A membrane oxygenator is a medical device which is used to improve the level of oxygen to and eliminate carbon dioxide from the blood.The machine is used in two key modes – (a) to oxygenate blood in prolonged life term support, which is known as extracorporeal membrane oxygenation, ECMO, and (b) to mimic the functioning of lungs during cardiopulmonary bypass (CPB). A membrane oxygenator machine does the responsibilities of both, the heart, i.e., to pump blood, and the lungs, i.e., to oxygenate the blood. The oxygenator machine comprises of a thin gas permeable membrane that separates the flow of gas and blood in the CPB unit. The oxygen permeates from the gas side into the blood, and the carbon dioxide permeates into the gas from the blood in order to dispose.For instance,during an open heart surgery, the blood that is getting back to the heart is deviated with the help of the oxygenator machine prior to returning to the arterial circulation.This is also referred to as the heart lung machine. According to study done on membrane oxygenator industry market, the American Lung Association presented a data in which COPD was identified as the third contributing cause of deaths in the United States in 2013. John H. Gibson invented the first ever membrane oxygenator machine which was successfully put to use in open-heart surgery in 1953.

Membrane oxygenation in ECMO:

ECMO is widely used in children who are ill owing to severe, but reversible, breathing (lungs) or heart problems. Since ECMO takes the work of heart and lungs, the patient is able to recuperate.With the help of ECMO babies’ heart and lungs getmore time to develop. Also, ECMO is a link between before and after a heart surgery. This procedure is only required during extreme conditions, and when all the other supportive measure have not proven efficient. Without the help of membrane oxygenator, the rate of survival is such cases is around 20 percent or even less, and with the machine it escalates to 60 percent.

Extreme conditions which require ECMO are:

• Heart defect at birth
• Meconium aspiration syndrome (MAS)
• Acute air leak problems
• Congenital diaphragmatic hernia (CDH)
• Severe pneumonia
• High blood pressure in the arteries of the lungs (PPHN)
• Post-surgery while recuperating

There are two types of extracorporeal membrane oxygenation –

• Veno-venous (VV) ECMO: it takes blood from a specific vein and gives it back to the same vein. It is used in giving support to the lung function.
• Veno-arterial (VA) ECMO: it takes blood from a particular vein and returns it to the artery. This machine is more invasive than VV ECMO, and supports both, heart as well as lungs.
