Polycythemia Vera (PV) Overview
The current understanding of pathophysiology involves increased sensitivity to growth factors due to an abnormal hematopoietic cell clone, with signs and symptoms such as headache, dizziness, claudication, and thrombosis resulting from increased blood viscosity.
Diagnosis has evolved from the older PVSG criteria to the updated WHO 2016 guidelines, requiring either all three major criteria or two major plus one minor criterion, including elevated hemoglobin/hematocrit levels, bone marrow hypercellularity with trilineage proliferation, the presence of JAK2 mutations, and a low serum erythropoietin level as the minor criterion.
There is currently no cure for Polycythemia Vera; treatment goals are aimed at symptom relief and reducing the risk of complications including thrombosis, bleeding, and hematologic transformation, with no current means of preventing transformation into myelofibrosis or acute leukemia/myelodysplastic syndrome.
