What is Polycythemia Vera?

Polycythemia Vera (PV) is a myeloproliferative neoplastic disorder characterized by uncontrolled production of red blood cells, often accompanied by increased production of white blood cells and platelets. The disease is associated with abnormal hematopoietic cell clones and increased sensitivity to growth factors.
The majority of patients with Polycythemia Vera have a JAK2 mutation, with the JAK2V617F mutation observed in more than 90% of patients. This mutation results in constitutive activation of cytokine signaling and contributes to increased production of red blood cells and platelets.
Common signs and symptoms include headache, dizziness, claudication, thrombosis, and other complications associated with increased blood viscosity. Diagnosis involves assessment of hemoglobin and hematocrit levels, bone marrow examination, detection of JAK2 mutations, and evaluation of serum erythropoietin levels.
There is currently no cure for Polycythemia Vera. Treatment focuses on symptom relief and reducing the risk of complications such as thrombosis, bleeding, and hematologic transformation.

Bomedemstat: Merck Sharp & Dohme LLC
Bomedemstat (MK-3543) is an investigational small-molecule, irreversible LSD1 inhibitor being developed by Merck. LSD1 regulates the proliferation of hematopoietic stem cells and plays an essential role in cell differentiation and maturation. Bomedemstat is being evaluated across multiple myeloproliferative neoplasms, including essential thrombocythemia, myelofibrosis, and Polycythemia Vera. Currently, the drug is being evaluated in the Phase III stage of development for the treatment of Polycythemia Vera.

Flonoltinib: Chengdu Zenitar Biomedical Technology Co., Ltd
Flonoltinib is an orally bioavailable dual inhibitor of Janus-associated kinase 2 (JAK2) and FMS-like tyrosine kinase 3 (FLT3), with potential anti-inflammatory, immunomodulating, and antineoplastic activities. Upon oral administration, flonoltinib targets and inhibits JAK2 and FLT3 activity, preventing activation of the JAK/STAT signaling pathway and FLT3-mediated signaling. Currently, the drug is being evaluated in the Phase II stage of development for the treatment of Polycythemia Vera.

PRT12396: Prelude Therapeutics
PRT12396 is an oral, highly selective, allosteric inhibitor of the mutant JAK2V617F protein, specifically targeting the JH2 pseudokinase domain. Developed for the treatment of myeloproliferative neoplasms, PRT12396 is designed to selectively inhibit cells carrying the JAK2V617F mutation while sparing wild-type JAK2, with the potential to improve tolerability and reduce mutant allele burden. Currently, the drug is being evaluated in the Phase I stage of development for the treatment of Polycythemia Vera.
