What is Non-Cystic Fibrosis Bronchiectasis (NCFB)?

Non-Cystic Fibrosis Bronchiectasis (NCFB) is a chronic, progressive lung disease characterized by permanent and abnormal dilation of the bronchi. This structural damage leads to impaired mucociliary transport, chronic airway infection, and persistent inflammation. Unlike cystic fibrosis, NCFB is a heterogeneous condition that can result from various infectious, inflammatory, immune-related, and airway-clearance abnormalities.
NCFB most frequently affects older adults and is associated with increased morbidity and reduced quality of life. The hallmark symptoms include a persistent daily cough and chronic mucus production. Patients may also experience chest pain, shortness of breath, wheezing, fatigue, recurrent fever or chills, and, in some cases, hemoptysis.
The disease is driven by a self-perpetuating cycle involving impaired mucociliary clearance, chronic infection, and persistent inflammation. Accumulation of mucus allows pathogens to persist and triggers predominantly neutrophilic inflammation. The resulting inflammatory mediators and proteolytic enzymes progressively damage the bronchial walls, causing irreversible airway dilation and further impairment of mucus clearance.
NCFB may arise from a wide range of underlying conditions, including autoimmune diseases, immunodeficiency disorders, abnormalities of ciliary function, allergic bronchopulmonary aspergillosis, chronic pulmonary aspiration, COPD, alpha-1 antitrypsin deficiency, inflammatory bowel diseases, and recurrent or severe respiratory infections such as pneumonia, pertussis, tuberculosis, and fungal infections.
Diagnosis generally involves clinical evaluation, medical and family history, blood testing, imaging, lung function testing, and sputum cultures. High-resolution CT is the most common and definitive imaging method used to confirm bronchiectasis. Management focuses on controlling symptoms, preventing recurrent infections and exacerbations, improving mucus clearance, and addressing underlying conditions. Treatment may include antibiotics, inhaled therapies, hydration, chest physiotherapy, oxygen therapy when required, and surgery in selected severe cases.

HSK31858 is a novel, oral, irreversible dipeptidyl peptidase-1 (DPP-1) inhibitor being developed by Haisco Pharmaceutical Group for neutrophil-mediated chronic inflammatory diseases, particularly Non-Cystic Fibrosis Bronchiectasis.
