What is Mucopolysaccharidosis III?

Mucopolysaccharidosis type III (MPS III), also known as Sanfilippo syndrome, is a rare genetic condition that causes progressive and severe neurological deterioration. It results from the deficiency of an enzyme required to break down and recycle heparan sulfate. The accumulation of heparan sulfate causes progressive cellular damage, particularly within the central nervous system. MPS III has four subtypes-MPS IIIA, IIIB, IIIC, and IIID-each associated with mutations affecting a different enzyme and gene.
The disease is primarily characterized by developmental delay, progressive intellectual disability, behavioral abnormalities, aggression, hyperactivity, sleep disturbances, and neurodegeneration. Symptoms commonly begin between 2 and 6 years of age, while physical manifestations may include coarse facial features, joint stiffness, and speech delays. The severity and rate of progression vary across subtypes and even among affected siblings.
The enzyme deficiency causes incomplete degradation and progressive accumulation of heparan sulfate within lysosomes. This accumulation disrupts cellular function, causes lysosomal enlargement, and triggers inflammatory and oxidative pathways that contribute to neuronal damage and death. Diagnosis involves enzyme testing and molecular genetic analysis to identify pathogenic variants in genes including GNS, HGSNAT, NAGLU, and SGSH. Currently, no therapies are clinically available to treat the primary manifestations of MPS III, and management remains primarily supportive.

DNL126: Denali Therapeutics
DNL-126 is an investigational enzyme replacement therapy being developed for patients with Sanfilippo syndrome type A. The deficient SGSH enzyme has been engineered to cross the blood-brain barrier by binding to the transferrin receptor on blood vessels. This approach is intended to address disease pathology in both the brain and body. Currently, the drug is being evaluated in the Phase I/II stage of development for the treatment of Mucopolysaccharidosis III.

JR441: JCR Pharmaceuticals Co., Ltd.
JR-441 is an investigational enzyme replacement therapy being developed for the treatment of Mucopolysaccharidosis Type IIIA (MPS IIIA), or Sanfilippo Syndrome Type A. JR-441 is a recombinant fusion protein consisting of heparan N-sulfatase, the enzyme missing or dysfunctional in MPS IIIA patients, fused to a humanized monoclonal antibody that targets the human transferrin receptor. GC1130A is a first-in-class treatment that uses GC Biopharma's proprietary platform to produce concentrated, high-quality recombinant protein administered via ICV injection to bypass the blood-brain barrier.
