What is Sickle Cell Disease?
Sickle Cell Disease (SCD) is a group of inherited conditions that affect red blood cells, with sickle cell anemia being the most serious type. Sickle cells are destroyed rapidly in people with the disease, causing anemia. The sickle cells can also block blood flow through vessels, resulting in pain episodes, acute chest syndrome, stroke, and damage to organs including the spleen, kidneys, and liver.
Common symptoms associated with Sickle Cell Disease include anemia, joint pain, acute chest syndrome, swelling of the hands and feet, stroke, jaundice, priapism, dizziness, fatigue, low oxygen levels, and malaise.
Sickle Cell Disease affects hemoglobin, the molecule in red blood cells responsible for delivering oxygen to cells throughout the body. When oxygen levels are insufficient, sickle hemoglobin becomes less soluble and promotes polymer formation, increasing blood viscosity and contributing to the formation of sickled red blood cells.
Sickle Cell Disease is inherited in an autosomal recessive pattern. Diagnosis can be performed through blood testing and is routinely included in newborn screening. Treatment may include lifestyle measures, medical screening, and interventions designed to prevent complications. Existing treatments include hydroxyurea, L-glutamine, voxelotor, and crizanlizumab, while several other therapies continue to undergo clinical development.
Emerging Sickle Cell Disease Drug Profiles
Inclacumab: Pfizer
Inclacumab is a fully human IgG4 anti-P-selectin monoclonal antibody being developed to reduce vaso-occlusive crises (VOCs) in patients with Sickle Cell Disease. P-selectin is a protein involved in cell adhesion and represents a clinically validated target for reducing pain associated with VOCs.
Inclacumab was originally developed by Global Blood Therapeutics, which was acquired by Pfizer. The drug is currently in the Phase III stage of development for the treatment of patients with Sickle Cell Disease.
Renizgamglogene autogedtemcel: Editas Medicine
Renizgamglogene autogedtemcel (reni-cel), formerly known as EDIT-301, is an experimental gene-editing medicine being investigated for severe Sickle Cell Disease and transfusion-dependent beta thalassemia. It consists of patient-derived CD34+ hematopoietic stem and progenitor cells edited at the gamma-globin gene (HBG1 and HBG2) promoters using AsCas12a.
The edited cells are designed to increase fetal hemoglobin production, with the potential to provide a one-time and durable treatment benefit for patients with severe Sickle Cell Disease. The drug has completed Phase III clinical development for patients with Sickle Cell Disease.
RG 6107: Roche
RG 6107, also known as SKY59, is a recycling antibody discovered by Chugai that inhibits the complement component C5. It is designed to inhibit the cleavage of C5 into C5a and C5b, thereby suppressing complement activation.
The antibody uses proprietary engineering technologies intended to extend its half-life and is being developed for subcutaneous self-injection. Currently, the drug is in the Phase II stage of development for the treatment of patients with Sickle Cell Disease.
