What is Frontotemporal Dementia?

Frontotemporal Dementia (FTD) refers to a group of related conditions resulting from the progressive degeneration of the frontal and temporal lobes of the brain. These areas play an important role in behavior, language, and decision-making abilities. FTD is considered a young-onset dementia, commonly occurring between the ages of 45-65 years, and is one of the most common dementias in people under 60 years of age.
Frontotemporal Dementia can present through different clinical syndromes depending on the underlying pathology. Abnormal aggregates of proteins such as TDP-43 and tau are important pathological hallmarks. Genetic factors also play a significant role, with inherited mutations involving genes such as MAPT, GRN, and C9orf72 associated with familial forms of the disease.
Symptoms may include progressive changes in personality and behavior, loss of inhibition, compulsive behaviors, difficulties with language and communication, impaired judgment, and changes in social conduct. Diagnosis generally involves clinical assessment, neurological examination, brain imaging, and genetic testing where appropriate.
There is currently no cure or specific disease-modifying treatment for Frontotemporal Dementia. Management primarily focuses on controlling symptoms and supporting patients and caregivers. Emerging therapies are being investigated with the aim of addressing underlying pathological mechanisms and slowing disease progression.

Emerging Frontotemporal Dementia Drug Profiles

TPN-101: Transposon Therapeutics
TPN-101 specifically inhibits the LINE-1 reverse transcriptase that promotes LINE-1 replication. LINE-1 elements are a class of retrotransposable elements that in humans are uniquely capable of replicating and moving to new locations within the genome. When this process becomes dysregulated, LINE-1 reverse transcriptase can drive overproduction of LINE-1 DNA, triggering innate immune responses that contribute to neurodegenerative, autoimmune, and aging-related disease pathology.
Currently, TPN-101 is being evaluated in the Phase II stage of development for the treatment of Frontotemporal Dementia.

AVB-101: AviadoBio
AVB-101 is a potential one-time gene therapy designed to stop disease progression by delivering a functional copy of the GRN gene to restore appropriate progranulin levels in affected areas of the brain. The drug is delivered using a minimally invasive, stereotactic neurosurgical procedure directly to the thalamus.
The targeted delivery approach is designed to deliver treatment directly to the brain while minimizing systemic exposure. In November 2023, the U.S. Food and Drug Administration granted Fast Track designation to AVB-101. Currently, the drug is being evaluated in the Phase I/II stage of development for the treatment of Frontotemporal Dementia.

PBFT02: Passage Bio
PBFT02 is a gene therapy product candidate using an AAV1 viral vector to deliver modified DNA encoding the granulin gene (GRN) to a patient's cells. The vector is delivered directly to the cerebrospinal fluid through a single injection into the cisterna magna.
The objective of this vector and delivery approach is to provide higher-than-normal levels of progranulin protein to the central nervous system and address progranulin deficiency in patients with GRN gene mutations. Currently, the drug is being evaluated in the Phase I/II stage of development for the treatment of Frontotemporal Dementia.
