What is Hypoglycemia?
Hypoglycemia is often defined by a plasma glucose concentration below 70 mg/dL, although signs and symptoms may not occur until plasma glucose concentrations drop below 55 mg/dL. Glucose is the primary metabolic fuel for the brain, and interruption of the glucose supply can lead to potentially serious complications.
Hypoglycemia, or low blood sugar, can cause rapid symptoms including shakiness, sweating, fast heartbeat, hunger, dizziness, anxiety, and confusion. Mild symptoms can often be treated with quick-acting carbohydrates, while severe untreated cases may lead to seizures, loss of consciousness, or coma.
The causes of Hypoglycemia vary depending on whether a person has diabetes. In patients with diabetes, it is primarily associated with pharmacologic therapies such as insulin, sulfonylureas, and meglitinides. In non-diabetic individuals, causes may include iatrogenic factors, alcohol consumption, critical illnesses, hormonal deficiencies, and tumors such as insulinoma.
Diagnosis is based on confirmation of Whipple's triad along with laboratory evaluation of insulin, proinsulin, and C-peptide levels during an episode. Management depends on severity and may include oral glucose, intravenous dextrose, or glucagon, followed by measures to maintain euglycemia.
Emerging Hypoglycemia Drug Profiles
Zimislecel: Vertex Pharmaceuticals Incorporated
Zimislecel, formerly known as VX-880, is an allogeneic stem cell-derived, fully differentiated, insulin-producing islet cell therapy manufactured using proprietary technology. It is being evaluated for patients who have type 1 diabetes with impaired hypoglycemic awareness and severe hypoglycemia. Zimislecel is being investigated as an infusion into the hepatic portal vein and requires anti-rejection medication to protect the islet cells from immune rejection. Currently, the drug is being evaluated in the Phase III stage of development for the treatment of Hypoglycemia.
Imapextide: MBX Biosciences
Imapextide, also known as MBX 1416, is an investigational therapy being developed as a potential treatment for post-bariatric hypoglycemia (PBH). MBX 1416 is a long-acting GLP-1 receptor antagonist designed to block pathological increases in GLP-1 released following a meal, which can increase hyperinsulinemia and result in hypoglycemia. By inhibiting GLP-1-induced hyperinsulinemia, Imapextide is designed to reduce insulin secretion and increase blood glucose, potentially reducing the frequency and severity of hypoglycemic events with once-weekly administration. Currently, the drug is being evaluated in the Phase II stage of development for the treatment of Hypoglycemia.
ZT-01: Zucara Therapeutics
ZT-01 is a first-in-class, once-daily therapeutic being developed to prevent insulin-induced hypoglycemia in patients using insulin therapy. ZT-01 is designed to inhibit somatostatin, a pancreatic hormone that impairs the glucagon response to hypoglycemia in people with insulin-dependent diabetes. By restoring glucagon secretion, ZT-01 is designed to prevent hypoglycemia and potentially improve diabetes disease management and quality of life. Currently, the drug is being evaluated in the Phase II stage of development for the treatment of Hypoglycemia.
