What is Scleroderma?
Scleroderma, also known as systemic sclerosis, is a chronic autoimmune disorder characterized by excessive collagen production that leads to fibrosis of the skin and internal organs. The disease affects connective tissues and can involve multiple organ systems, including the lungs, heart, kidneys, and gastrointestinal tract. It is associated with vascular dysfunction, chronic inflammation, and autoantibody production. Scleroderma occurs predominantly in women, with a female-to-male ratio of approximately 4:1, and affects an estimated 150-300 individuals per million population.
Raynaud's phenomenon is among the most common early manifestations and occurs in approximately 90% of patients. Internal organ involvement, particularly affecting the lungs and gastrointestinal tract, occurs in approximately 30-50% of patients and contributes significantly to disease morbidity and mortality. Despite therapeutic advances, there is currently no cure for Scleroderma, and treatment remains focused on symptom management and prevention of disease progression.

Divozilimab: BIOCAD
Divozilimab (BCD-132) is a humanized anti-CD20 monoclonal antibody being developed by BIOCAD for the treatment of systemic sclerosis. The therapy selectively binds CD20-positive B lymphocytes and promotes B-cell depletion through antibody-dependent cellular cytotoxicity and complement-mediated cytotoxicity. This mechanism aims to reduce pathogenic immune activity and fibrosis associated with Scleroderma. Results from a randomized, double-blind, placebo-controlled Phase III study demonstrated a statistically significant reduction in skin fibrosis severity. Patients receiving Divozilimab achieved a mean modified Rodnan Skin Score (mRSS) improvement of -5.8 points compared with -2.7 points in the placebo group. The therapy also demonstrated stabilization of lung function and a favorable safety profile, highlighting its potential as a promising treatment option for systemic sclerosis.
CABA-201: Cabaletta Bio
CABA-201 is an investigational Anti-CD19 CAR-T cell therapy being developed by Cabaletta Bio for patients with systemic sclerosis. The therapy is currently in Phase I/II clinical development and is designed to eliminate autoreactive B cells responsible for driving autoimmune pathology. By resetting the immune system, CABA-201 has the potential to provide durable disease control in patients with severe forms of systemic sclerosis.
BI 685509: Boehringer Ingelheim
