Decision-Making Under Uncertainty: Why Leadership Needs More Than Just Data

Vlotho, June 9, 2026. When making decisions under uncertainty, one does not simply calculate probabilities. A recent meta-analysis in the journal Frontiers in Neuroscience, which summarizes 76 imaging studies involving 4,186 participants, shows that when making decisions under uncertainty, rational thought and gut feeling converge in two brain regions: the anterior insula and the anterior cingulate cortex. Executives make decisions every day whose outcomes they cannot be certain of. For a long time, it was assumed that the brain processes such uncertainty primarily via the amygdala and medial prefrontal cortex--that is, via the classic fear and evaluation system. The new meta-analysis corrects this picture. Consistently activated across dozens of studies are the anterior insula, the inferior frontal gyrus, and the inferior parietal lobe. The insula and cingulate cortex act as hubs that integrate physical signals and cognitive evaluation.

This has an inconvenient implication for practice. Pure data analysis falls short because it addresses only the cognitive half. The bodily signal processed by the insula cannot be rationalized away; rather, it is an integral part of the decision. Anyone who wants to develop decision-making skills must train both sides: the internal reaction and the reasoning.
