Chronic Refractory Cough Overview
Chronic refractory cough (CRC) is a persistent cough lasting longer than eight weeks that remains unresolved despite thorough evaluation and treatment of underlying causes such as asthma, gastroesophageal reflux disease, or upper airway cough syndrome. It is increasingly recognized as a disorder of cough hypersensitivity, in which heightened sensitivity of airway sensory nerves leads to an exaggerated cough reflex triggered by otherwise innocuous stimuli such as talking, cold air, or mild irritants.
CRC is characterized primarily by a persistent, dry cough lasting more than eight weeks that does not respond to standard treatments for common underlying conditions. Patients often experience frequent coughing bouts triggered by everyday stimuli such as talking, laughing, cold air, strong smells, or eating. A hallmark feature is a heightened cough reflex accompanied by sensations of throat irritation, tickling, or an urge-to-cough feeling (laryngeal paresthesia). Additional symptoms may include hoarseness, throat clearing, and a sensation of something stuck in the throat (globus).
Chronic refractory cough is primarily understood as a disorder of cough hypersensitivity, in which the neural pathways regulating the cough reflex become dysregulated. Repeated airway inflammation or injury-often from prior respiratory conditions-leads to sensitization of peripheral sensory nerves (particularly vagal afferents) in the airway epithelium. This results in an exaggerated response to otherwise harmless stimuli such as talking, cold air, or mild irritants. At the molecular level, upregulation of receptors such as transient receptor potential (TRP) channels and purinergic P2X3 receptors contributes to heightened neuronal excitability.
The etiology of CRC is typically multifactorial and often follows common causes of chronic cough that have either been treated or are no longer active. These include conditions such as asthma, gastroesophageal reflux disease (GERD), upper airway cough syndrome (postnasal drip), and respiratory infections, which may initially trigger the cough reflex. Environmental exposures (e.g., pollutants, smoke), use of medications like ACE inhibitors, and laryngeal dysfunction can also contribute.
The diagnosis of CRC is largely one of exclusion and follows a systematic evaluation of chronic cough lasting more than eight weeks. Clinicians begin with a detailed clinical history, medication review (e.g., ACE inhibitors), and assessment for common underlying causes such as asthma, gastroesophageal reflux disease (GERD), and upper airway cough syndrome. Standard investigations may include chest imaging (X-ray or CT), spirometry or lung function testing, and, when indicated, tests for airway inflammation or reflux. If the cough persists despite appropriate treatment of these conditions and no alternative cause is identified, the diagnosis of CRC is considered.
Chronic Refractory Cough Emerging Drugs Profile
• Camlipixant: GSK
Camlipixant (formerly BLU-5937) is an investigational, oral, small-molecule P2X3 receptor antagonist designed to treat refractory chronic cough (RCC) and other cough hypersensitivity disorders. The P2X3 receptor is an ATP-gated ion channel highly expressed on sensory nerve fibers in the airways, particularly vagal C-fibers and Aδ-fibers that mediate the cough reflex. In chronic refractory cough, these sensory neurons become hypersensitive, responding excessively to normally innocuous stimuli. By selectively antagonizing P2X3 receptors, Camlipixant reduces the hyperexcitability of these sensory neurons, thereby dampening the exaggerated cough reflex without affecting normal protective cough mechanisms.
Phase IIb studies (SOOTHE trial) have demonstrated that Camlipixant effectively reduces 24-hour cough frequency by approximately 34% at both 50-mg and 200-mg doses compared to placebo. Importantly, the drug has shown a favorable safety profile with notably low rates of taste alteration (dysgeusia), a common adverse effect associated with other P2X3 antagonists. This improved tolerability profile may enhance patient compliance and quality of life during long-term treatment. The oral formulation provides convenient once-daily or twice-daily dosing, making it practical for chronic disease management.
Camlipixant represents a targeted, mechanism-based approach to treating CRC by addressing the underlying neuronal hypersensitivity rather than simply suppressing cough through sedation or central nervous system effects. Currently, the drug is being evaluated in the Phase III stage of its development for the treatment of Chronic Refractory Cough, with the potential to become a first-line therapy for this challenging condition.
• HW091077: Hubei Bio-Pharmaceutical Industrial Technological Institute Inc.
HW091077 is an investigational therapeutic candidate being developed by Hubei Bio-Pharmaceutical Industrial Technological Institute Inc. for the treatment of chronic refractory cough. While specific mechanistic details have not been fully disclosed publicly, HW091077 represents one of the emerging therapeutic approaches aimed at addressing the underlying pathophysiology of CRC, particularly targeting the cough hypersensitivity pathways that characterize this condition.
The development of HW091077 reflects the growing recognition within the pharmaceutical industry that chronic refractory cough is a distinct neurological disorder requiring targeted intervention beyond traditional cough suppressants. The compound is designed to provide relief for patients who have not responded to conventional therapies for common cough etiologies such as asthma, GERD, or upper airway cough syndrome.
HW091077 is being evaluated in clinical trials to assess its efficacy in reducing cough frequency and severity, as well as improving quality of life measures such as sleep disruption, social embarrassment, and daily functioning impairment caused by persistent coughing. Safety and tolerability profiles are also being carefully monitored throughout the development process. The drug is currently progressing through clinical development stages for chronic refractory cough indication, representing an important addition to the limited therapeutic landscape for this underserved patient population.
• Haduvio: Trevi Therapeutics
Haduvio is an investigational neuromodulator being developed by Trevi Therapeutics for the treatment of chronic refractory cough and other conditions characterized by neuronal hypersensitivity. The therapeutic agent is designed to target the dysregulated neural pathways that underlie the exaggerated cough reflex seen in patients with chronic refractory cough, particularly those who have failed standard treatments for identifiable underlying causes.
Chronic refractory cough is increasingly understood as a neuropathic disorder in which peripheral and central sensitization of cough-related neural pathways leads to heightened responsiveness to normally innocuous stimuli. Haduvio's mechanism appears to modulate these hypersensitive neural circuits, potentially acting on multiple levels of the cough reflex arc-from peripheral sensory nerve terminals in the airways to central processing centers in the brainstem.
The development of Haduvio addresses a significant unmet medical need, as patients with chronic refractory cough often experience substantial impairment in quality of life, including sleep disturbance, social isolation, urinary incontinence, rib fractures, and psychological distress. Current treatment options remain limited, with off-label use of medications such as gabapentin, pregabalin, and low-dose opioids showing variable efficacy and tolerability concerns.
Haduvio is being evaluated in clinical trials to assess its ability to reduce cough frequency, improve cough-related quality of life measures, and demonstrate acceptable safety and tolerability profiles for chronic use. The drug represents part of a new generation of mechanism-based therapies specifically designed for cough hypersensitivity disorders. Currently, Haduvio is progressing through clinical development for chronic refractory cough, offering hope to patients suffering from this debilitating and often socially isolating condition.
