What is Alopecia Areata?
Alopecia Areata is a chronic autoimmune disorder characterized by sudden, non-scarring hair loss caused by immune-mediated destruction of hair follicles. The condition commonly presents as patchy hair loss on the scalp but may also affect eyebrows, eyelashes, facial hair, and other body regions.

In severe cases, Alopecia Areata may progress to alopecia totalis or alopecia universalis, leading to complete scalp or body hair loss. The disease is associated with significant psychosocial burden, emotional distress, and reduced quality of life.

According to the National Alopecia Areata Foundation, more than 80% of patients show signs of the disease before the age of 40, while approximately 40% experience symptoms before the age of 20.

Symptoms may include sudden hair loss, nail abnormalities such as pitting or ridging, and sensations of itching or tingling before hair shedding occurs. Diagnosis typically involves clinical examination, scalp assessment, blood tests, and, in certain cases, scalp biopsy to distinguish among various alopecia subtypes.

Emerging Alopecia Areata Drug Profiles

Rezpegaldesleukin: Nektar Therapeutics
Rezpegaldesleukin is an engineered IL-2 pathway agonist designed to selectively expand regulatory T cells (Tregs) while minimizing activation of pro-inflammatory immune responses. The therapy aims to restore immune tolerance at the hair follicle level and reduce autoimmune-mediated follicular destruction.

The therapy is currently being evaluated in the Phase II REZOLVE-AA clinical trial for severe-to-very severe Alopecia Areata. The study demonstrated clinically meaningful activity with encouraging SALT score reduction and a favorable safety profile.

In July 2025, the US FDA granted Fast Track designation to rezpegaldesleukin for the treatment of severe-to-very severe Alopecia Areata in adults and pediatric patients aged 12 years and older.

Upadacitinib: AbbVie
Upadacitinib (RINVOQ®) is a selective JAK1 inhibitor being developed by AbbVie for the treatment of severe Alopecia Areata. The therapy is currently in Phase III clinical development and is administered orally.

The drug has demonstrated promising efficacy in improving scalp hair regrowth among patients with high baseline SALT scores and represents one of the most advanced late-stage pipeline candidates in the Alopecia Areata therapeutic landscape.

• In August 2025, AbbVie announced positive topline results from the second pivotal Phase III UP-AA clinical study evaluating upadacitinib (RINVOQ®) in adult and adolescent patients with severe Alopecia Areata.
• HCW Biologics continues to advance HCW9302 through Phase I clinical evaluation for Alopecia Areata, targeting immune regulatory pathways through recombinant fusion protein technology.
