In the "SCENIHR Report", the European Commission calls for novel, improved materials for uro-gyneacological mesh implants.
PVDF (polyvinylidene fluoride) is explicitly mentioned as a promising and new synthetic material that is well established in hernia surgery.
In April 2016, PVDF was first mentioned in the guideline Diagnosis and Therapy of Genital Descensus in Women of the DGGG (German Society of Gynaecologists and Obstetricians), OEGGG (Austrian Society of Gynaecologists and Obstetricians) and the SGGG (Swiss Society of Gynaecologists and Obstetricians).
The final version of the European Commission's "SCENIHR Report" on emerging and newly identified health risks in uro-gyneacological surgery was published recently. Among other aspects, it analysed the safety of mesh implants in uro-gyneacological surgery.
The report shows the results of an intensive discussion of the previously published proposal of the European Commission that incorporates feedback from general practitioners, scientists and professional associations from all over Europe.
A general assessment of risks of a given indication, recommended surgical procedures, level of knowledge required by the surgeon and especially the mesh material used in incontinence and prolapse surgery, was critically surveyed.
The report calls for the development of novel techniques and materials for meshes and their introduction (up until recently meshes were mainly manufactured from polypropylene) as well as, in this case, specifically refers to the recommendations of the IUGA (Slack et al.).A statement issued by the IUGA on this report explicitly highlights PVDF for its reduced foreign body reaction, increased elasticity and stability compared to other common materials used in this clinical context. Therefore, the essential requirements of the SCENIHR report are met. Additionally required research on the use of PVDF in uro-gyneacology are in progress, both in international and multicentre clinical studies.
The withdrawal of the US company Astora (formerly AMS), as well as the discontinuation of polypropylene mesh production at several other mesh companies, are indications of how problematic the use of this mesh material is in the medium to long term.
PVDF not only reduces the foreign body reaction but also exhibits superior long-term stability and durability. Therefore, surgical suture material made from PVDF has been used for particularly delicate applications: cardiac surgery and eye surgery. In hernia surgery, PVDF has also been used successfully for over a decade.
