When there are cases of excess skin removed, only one incision is made by the surgeon around the edge of the areola, sometime also referred to as donut procedure, to reposition the nipple higher up on the chest and decrease the areola size if desired by the patient. On the other hand when there is more removal of skin, often an incision is made by surgeon below the breast skin, known as the crease, and another one in a perpendicular angle running up to areaola, known as lollipop procedure, by which either way excess skin is eliminated and the breast tissue is positioned at higher on chest with contouring it as well. In both the cases, the nipple is generally repositioned to a new location for a natural perky look where the scar is effectively hidden by the breast crease and the areola’s edge hides the other. The scar typically fades to a thin line from the two connective incision sites.

If one chooses to have breast implants without a breast lift, one may have the same incision pattern as explained above. In some cases, there is insertion of saline implants through the incision site typically under the arm, around the areola edge or sometimes in rare cases, it is placed through the belly button and later filled after placement. The implant is inserted or placed into the space (pocket) made for it, either over or under the chest muscle.

The decision of placing these implants depends on careful thought on various considerations such as either to be positioned above or below the muscle, shape (low profile, high profile, teardrop and others), what type of implant (silicone or saline) and what size of the implant to inserted. If you are considering both a breast enlargement and breast lift, the proper timing for typically undertaking the surgeries should be performed at the same time. However, at some instances both the procedure is done as two separate surgeries. For either or both, it is best to wait until one gets finished with childbearing and is at a stable weight prior to undergoing the procedure.
