In the last recent years, there has been significant development in the facial surgery and not only there has been a growth in the quantity but in quality as well. This consistent growth has led to greater understanding of facial anatomy and the aging process.

Subcutaneous Facelift

Earlier the technique focused on the small local incisions which were near the hairline in the natural creases with excision of skin strips, but nowadays modern techniques involves combining the temporal hairline incisions and incisions that ends in the occipital scalp. The subcutaneous facelift is performed to tighten the loose facial skin and as well as to remove the excess fat without any additional damages to the deeper tissues. Though it is an easy process resulting in improvement in the lower face and neck, but the technique somewhere fails to eliminate the mid-face wrinkles and treat the structure deeper into the skin. In this surgery, the skin flap is placed under the wrinkles tension which leads to loss of effect and inherent elasticity of the skin and this is one reason why this method is not selected in the situations where the skin laxity is the main issue.

Deep Plane Techniques

Another technique to treat the face-lift in the early 90’s were the “deep plane rhyitidectomy”, where the problem was addressed by elevating the mid-facial soft tissues in a straight plane between the superficial and deep fascia creating a thick flap composed of skin and subcutaneous fat. The technique improved the nasolabial fold, neck and midface but it back fired as it required more technical dissection with increased risk to the facial nerve with longer surgical time.

Minimally Invasive And Noninvasive Techniques

After the popularization of the deep plane Rhytidectomy techniques in the late 80’s and early 90’s, there has been a change in the technique where the movement was more towards the minimally invasive techniques.

The minimally invasive techniques were replacing the extensive deep plane surgery because it required more operative time, longer recovery period and increased risks and complications during and post operation whereas the minimally surgery offered completely the opposite where they can be performed under local anesthesia, meaning one can go back home the same day as the day of surgery.

The other popular “non-surgical” technique with the minimally invasive technique market in the late 90’s was thread-lift. This technique involved placing of the subcutaneous barbed threads which were lift and trimmed at every point to make the facial skin smooth. But this technique did not last long as it did not have any long lasting effect with low longevity of the procedure.

In recent years, the marketing of less invasive and non – surgical techniques have been developed helping in offering skin tightening effects which include use of radiofrequency, laser and ultrasound energy (VASER). Though they involve dramatic surgical lifting, but they offer a variable alternative to those patients who are unwilling to undergo a surgery.
