Acquired Hemophilia A Overview
Acquired hemophilia A is a disorder of rare entity, resulting in spontaneous bleeding in individuals with no history of bleeding disorders. It is believed to be caused by spontaneous inhibition of clotting factor VIII by autoantibodies, and is usually associated with other autoimmune conditions. The hallmark of this condition is mucocutaneous bleeding leading to ecchymosis, melena, hematoma, or hematuria. The main form of hemophilia is hemophilia A, an X-linked disorder that mostly affects males but can also affect females.
Acquired hemophilia can be difficult to diagnose due to its rare occurrence. The most common symptoms include excessive bleeding into the skin, heavy and persistent bleeding after surgery, many large or deep bruises, blood in urine or stool, and nosebleeds of unknown cause. Acquired Hemophilia is suspected based on the clinical picture and confirmed by an abnormal coagulation test - a diagnosis should be considered with a recent onset of abnormal bleeding and an isolated prolongation of the activated partial thromboplastin time (aPTT). A factor VIII test is performed to measure the amount of factor in the blood, and a second specific test, the Bethesda inhibitor assay, determines the amount of antibody present.
The specific therapeutic procedures and interventions vary depending on numerous factors, including the specific symptoms present, the natural course of the disorder including its underlying cause, and the patient's age and overall health. Bypassing agents are the recommended first-line therapy due to their rapid action and high level of effectiveness, with recombinant activated factor VIII or activated prothrombin complex concentrate presently available as bypassing agents. Other treatment options involve the use of corticosteroids alone or in combination with cyclophosphamide.
Acquired Hemophilia A Emerging Drugs Profile
*
Marstacimab: Pfizer Pfizer has initiated a Phase 3 study of marstacimab, an investigational drug being evaluated for the treatment of severe hemophilia A or B with or without inhibitors. Marstacimab is an anti-tissue factor pathway inhibitor (anti-TFPI) in development as a prophylactic treatment to prevent or reduce the frequency of bleeding episodes in individuals with severe hemophilia A or B.
*
Mim8: Novo Nordisk Mim8 is in Phase II of clinical development as a novel, next-generation FVIII mimetic for subcutaneous prophylactic treatment in people with Hemophilia A (HA), with and without inhibitors. Mim8 is a fully human bispecific antibody bridging FIXa and FX.
*
Eptacog Beta (LR769): Hema Biologics Eptacog Beta (LR769) is a recombinant form of human factor VIIa under FDA review for the treatment of bleeding episodes in hemophilia A or B patients with inhibitors. Currently, two additional Phase 3 trials are investigating the use of eptacog beta in pediatric and surgical patients with hemophilia A or B with inhibitors.
