What is Primary Biliary Cholangitis (PBC)?

Primary Biliary Cholangitis (PBC) is a chronic, progressive autoimmune disease that primarily affects the bile ducts in the liver. It leads to inflammation and gradual destruction of the small bile ducts, causing bile to accumulate in the liver, also known as cholestasis. Over time, this can result in liver damage, fibrosis, and eventually cirrhosis.
Although the exact cause of PBC remains unclear, it predominantly affects middle-aged women and is considered an autoimmune disorder. Genetic and environmental factors, such as infections or toxic exposures, may contribute to its onset.
Early stages of PBC may be asymptomatic, but as the disease progresses, common symptoms include persistent fatigue, pruritus (itching), and dry eyes or mouth. Advanced stages can lead to jaundice, dark urine, pale stools, and swelling in the abdomen or legs due to fluid retention. Complications include vitamin deficiencies, osteoporosis, and an increased risk of liver failure or hepatocellular carcinoma.
PBC is typically diagnosed through a combination of blood tests, imaging studies, and sometimes liver biopsy. Elevated levels of alkaline phosphatase (ALP) and other liver enzymes suggest bile duct dysfunction. The presence of antimitochondrial antibodies (AMA) in the blood is a key diagnostic marker and is present in over 90% of cases.
There is currently no cure for PBC, but treatments aim to slow disease progression, alleviate symptoms, and manage complications. Ursodeoxycholic acid (UDCA) is the first-line medication that improves bile flow and liver function, while other therapeutic approaches are being explored for patients with an inadequate response or intolerance to standard therapy.

Emerging Primary Biliary Cholangitis Drug Profiles

Saroglitazar Magnesium: Zydus Therapeutics
Saroglitazar is a novel Peroxisome Proliferator-Activated Receptor (PPAR) α/γ agonist currently under investigation for the treatment of patients with Primary Biliary Cholangitis. Its distinct biological profile targets both bile acid toxicity and liver inflammation.
The US FDA has granted Orphan Drug Designation (ODD) and Fast Track Designation (FTD) to saroglitazar Mg for PBC. The US FDA has also granted it Priority Review with a target action date of November 27, 2026, for adults who are intolerant to or inadequately respond to standard UDCA therapy.
The NDA is supported by the EPICS-III trial Phase III results, a randomized, double-blind, placebo-controlled study evaluating saroglitazar in adult patients with PBC who had an inadequate response to or intolerance of UDCA.

Setanaxib: Calliditas Therapeutics
Setanaxib (GKT831) is an investigational, first-in-class oral NADPH oxidase (NOX) 1/4 inhibitor being developed for the treatment of Primary Biliary Cholangitis, particularly in patients with an inadequate response or intolerance to standard therapy.
By selectively inhibiting NOX1 and NOX4 enzymes, Setanaxib aims to reduce oxidative stress, inflammation, and fibrogenesis, which are key mechanisms involved in the progression of PBC. The therapy has demonstrated improvements in biomarkers associated with cholestasis and liver injury and is currently being evaluated in Phase II clinical development for PBC.

DF-5112: Dragonfly Therapeutics
DF-5112 is an investigational monoclonal antibody targeting C-C chemokine receptor 6 (CCR6), being developed by Dragonfly Therapeutics for immune-mediated diseases, including Primary Biliary Cholangitis.
By antagonizing CCR6, DF-5112 is designed to inhibit the migration and activation of pathogenic Th17 cells and other CCR6-expressing immune cells implicated in chronic inflammation and autoimmune-mediated bile duct injury.
This targeted immunomodulatory approach aims to reduce hepatic inflammation while potentially slowing disease progression in patients with PBC. As of 2026, DF-5112 is in Phase I clinical development.
