Familial Adenomatous Polyposis Overview
Familial adenomatous polyposis (FAP) is a rare inherited autosomal dominant disorder caused primarily by mutations in the APC gene. It is characterized by the development of hundreds to thousands of adenomatous polyps in the colon, typically beginning during adolescence. If left untreated, these polyps almost inevitably progress to colorectal cancer, making early diagnosis, surveillance, and preventive management essential. FAP is also associated with an increased risk of several extracolonic malignancies, including duodenal, gastric, thyroid, and liver cancers. Familial adenomatous polyposis (FAP) is primarily caused by germline mutations in the APC (adenomatous polyposis coli) tumor suppressor gene, which is inherited in an autosomal dominant manner. These mutations disrupt the regulation of cell growth and differentiation in the intestinal epithelium, leading to the formation of numerous adenomatous polyps in the colon. Approximately 20-30% of cases arise from de novo APC mutations in individuals with no family history of the disease.

Familial Adenomatous Polyposis Emerging Drugs Profile
• eRapa: Biodexa Pharmaceuticals
eRapa is a proprietary oral tablet formulation of rapamycin (sirolimus), a potent mTOR inhibitor. mTOR plays a key role in regulating cellular metabolism, growth, and proliferation and is often activated during tumorigenesis. Overexpression of mTOR in FAP polyps provides a strong rationale for using eRapa as a targeted therapy. The drug has received Orphan Designation in the US, with plans to pursue similar designation in Europe. Currently, the drug is in Phase III stage of its clinical study for the treatment of Familial adenomatous polyposis.
