What is Spinocerebellar Ataxias?
Spinocerebellar ataxia (SCA) is a rare, progressive, inherited neurodegenerative disorder that primarily affects the cerebellum but can involve other parts of the central nervous system. It is classified into over 40 subtypes based on genetic loci, with common types including SCA1, SCA2, SCA3, and SCA6.
Spinocerebellar Ataxias are characterized by the loss of voluntary muscle coordination, which can affect gait, speech, and eye movement. Symptoms typically worsen over time and can lead to significant disability. The disease may also affect areas such as the basal ganglia, cortex, and peripheral nerves.
The pathophysiology of Spinocerebellar Ataxia involves genetic mutations, abnormal protein aggregation, mitochondrial dysfunction, and dysregulated cellular processes. Many SCAs are caused by CAG repeat expansions that lead to the production of misfolded proteins called ataxins. Diagnosis involves family history, neurological assessments, imaging, and genetic testing.
There is currently no definitive cure for Spinocerebellar Ataxia, and treatment primarily focuses on alleviating symptoms. Management may include medications for seizures, tremors, and mood disorders, along with physical, occupational, and speech therapy. Emerging research is exploring approaches targeting misfolded mutant proteins, autophagy regulation, and RNA-based therapies.
Emerging Spinocerebellar Ataxias Drug Profiles
Troriluzole: Biohaven Pharmaceuticals, Inc.
Troriluzole is a New Chemical Entity (NCE) and third-generation novel prodrug that modulates glutamate, the most abundant excitatory neurotransmitter in the human body. Its primary mode of action is reducing synaptic levels of glutamate by increasing glutamate uptake from the synapse through enhanced expression and function of excitatory amino acid transporters on glial cells.
In February 2025, Biohaven announced that the U.S. Food and Drug Administration accepted its New Drug Application for troriluzole for the treatment of adult patients with Spinocerebellar Ataxia. In November 2025, Biohaven announced that it had received a Complete Response Letter from the FDA for the NDA seeking approval of VYGLXIA (troriluzole) for Spinocerebellar Ataxia. The drug is included in the Phase III stage of the Spinocerebellar Ataxias pipeline.
Stemchymal: Steminent US, Inc.
Stemchymal is an allogeneic stem cell therapy that uses mesenchymal stem cells isolated from the adipose tissue of healthy individuals. The cells are administered to recipients through intravenous infusion. The therapy has demonstrated therapeutic effects in vivo, with infusion shown to partially restore motor function in SCA mouse models. Stemchymal has also been granted orphan drug designation in Japan. Currently, the drug is in the Phase II stage of development for the treatment of Spinocerebellar Ataxias.
VO659: Vico Therapeutics B. V.
VO659 is a clinical candidate directly targeting the CAG repeat expansion, the mutation that causes all polyglutamine diseases. It is an antisense oligonucleotide (ASO) designed to bind directly to CAG repeats, sterically block translation, and reduce mutant protein in an allele-preferential manner.
The CAG repeat expansion causes nine polyglutamine diseases, including Huntington's disease and various forms of Spinocerebellar Ataxia. VO659 is designed to reduce mutant protein while preserving essential normal gene function. Currently, the drug is in the Phase I/II stage of development for the treatment of Spinocerebellar Ataxias.
