It also raises the bar: competitors now face an evidence threshold rather than a marketing one.
Cardiogenic shock has become a systems problem, not a device problem. Hospitals have built multidisciplinary shock teams - interventional cardiology, heart failure, cardiac surgery, critical care - with escalation protocols and hub-and-spoke transfer networks. Vendors have adapted accordingly, selling training, hemodynamic monitoring integration and protocol support alongside hardware. The total artificial heart is back in serious development. Her Tuesday begins with a transfer request from a spoke hospital two counties out. She pulls up the referring team's hemodynamics on a shared platform, sees a cardiac index below 1.8 and rising lactate, and pages the interventional attending and the heart failure fellow into a five-minute call. The decision - transfer with temporary support initiated on arrival rather than escalating locally - is made before the patient leaves the referring ED.
By 11 a.m. she is negotiating with materials management, because the shock cart inventory has three of one catheter size and none of another, and the rep cannot deliver until Thursday. By 4 p.m. she is documenting escalation criteria for a quality review.
None of that appears in a market forecast. All of it determines whether the device gets used.
Winners and Losers
The clearest winners are the temporary support franchises. Companies whose portfolios sit in acute cardiogenic shock and protected PCI are riding both volume growth and improving evidence, and they have pricing latitude that durable-device makers largely lack.
Integrated extracorporeal players win too. ECMO consoles, oxygenators, cannulae and circuits generate high-margin consumable pull-through long after the capital sale, and the installed base built out during the pandemic keeps generating disposable revenue.
Large advanced heart failure centres are winners in a different sense - volume concentration reinforces their referral position and their leverage with suppliers.
The losers are less obvious. Legacy intra-aortic balloon pump volume faces steady erosion wherever more powerful support is available and reimbursable, even as IABP survives on cost and familiarity. Business models built primarily on bridge-to-transplant LVAD implantation have been squeezed since US organ allocation changes reduced the incentive to bridge. Small community programmes without shock team infrastructure increasingly lose these patients to transfer - and lose the associated revenue with them.
Suppliers dependent on a single flagship platform carry concentrated regulatory risk, as recall history in this category has repeatedly demonstrated.
Regional Spotlight: North America
North America remains the largest and most commercially important region, and the reasons are structural rather than demographic. The United States has the densest network of certified advanced heart failure and transplant centres in the world, the highest per-capita implantation rates for durable LVADs, and reimbursement pathways - including established CMS coverage determinations for ventricular assist devices - that make these therapies financially viable for hospitals.
Clinical research concentration reinforces the advantage. The pivotal trials that define practice in this field have disproportionately enrolled at US and Canadian sites, which means American centres tend to adopt new platforms first and generate the registry data that drives adoption elsewhere.
The region also faces the sharpest version of the donor organ shortage relative to candidate volume, sustaining demand for destination therapy - permanent support for patients who will never be transplanted.
