What is Postherpetic Neuralgia?

Postherpetic neuralgia (PHN) is a common and painful complication that occurs after a shingles outbreak, caused by the reactivation of the varicella-zoster virus (VZV). It is characterized by persistent, burning or lancinating pain along a specific dermatomal pattern that lasts for months after the rash heals. Symptoms include persistent pain that lasts for three months or more after the rash has healed, which can be burning, sharp, jabbing, or deep and aching. Many individuals with PHN also experience heightened sensitivity, where even light touch, such as clothing on the skin, becomes unbearable. Additionally, some people may experience itching or numbness in the affected area.
The exact cause of Postherpetic Neuralgia is not fully understood, but it appears to involve both peripheral and central nervous system changes. Immune system activation, persistence of viral particles in nerve ganglia, nerve damage, inflammation, and central sensitization are believed to contribute to the development of the condition.
Treatment involves prevention, early treatment of acute herpes zoster, and symptom management. First-line treatments include oral tricyclic antidepressants, pregabalin, and the lidocaine 5% patch. Other options include capsaicin patches, NMDA antagonists, botulinum toxin, epidural injections, spinal cord stimulation, physical therapy, cognitive-behavioral therapy, acupuncture, and transcutaneous electrical nerve stimulation.
