Heparin-Induced Thrombocytopenia is a severe and potentially life-threatening immune-mediated adverse reaction triggered by the administration of heparin. Management involves immediate discontinuation of all forms of heparin and initiation of alternative non-heparin anticoagulants, such as direct thrombin inhibitors (e.g., argatroban, bivalirudin) or factor Xa inhibitors (e.g., fondaparinux, danaparoid), along with close monitoring of platelet counts and clinical signs of thrombosis and bleeding.

• ANGIOMAX (bivalirudin) - Sandoz
Bivalirudin is an inhibitor of thrombin used as an anticoagulant, reversibly binding thrombin at the catalytic site and anion-binding exosite to prevent fibrin formation and activation of Factor XIIIa and other coagulation factors. Administered intravenously, it is indicated for patients undergoing percutaneous coronary intervention (PCI), including those with Heparin-Induced Thrombocytopenia or Heparin-Induced Thrombocytopenia with thrombosis syndrome.
• Argatroban - Sandoz/Fresenius Kabi/Hikma Pharma
A synthetic direct thrombin inhibitor used in patients with thrombosis and Heparin-Induced Thrombocytopenia. It reversibly binds to the catalytic site of thrombin, blocking activation of clotting Factors V, VIII, and XII. Given intravenously and metabolized in the liver with a half-life of about 50 minutes, it may be suitable for patients with renal dysfunction.

A first-in-class, selective small molecule inhibitor of 12-lipoxygenase, blocking the 12-LOX pathway responsible for platelet activation in Heparin-Induced Thrombocytopenia.
