What is Liver Cirrhosis?
Liver Cirrhosis is a late-stage liver disease characterized by the replacement of healthy liver tissue with scar tissue, resulting in permanent liver damage and impaired liver function. The disease develops as a consequence of chronic liver injury and is histologically defined by regenerative nodules surrounded by fibrous bands. Progressive fibrosis eventually leads to portal hypertension and end-stage liver disease.
Activation of hepatic stellate cells plays a central role in the development of liver fibrosis and cirrhosis. The condition is associated with multiple pathological processes involving cytokines, microRNAs, endothelial dysfunction, inflammation, and extracellular matrix accumulation.
According to published research highlighted in the report, approximately 2.05 million new cases of liver cirrhosis were reported globally in 2019, with nearly 1.47 million deaths attributed to the disease, representing an increase of 8.1% compared to 2017.
Common symptoms include fatigue, weakness, jaundice, ascites, lower-extremity edema, appetite loss, weight loss, bleeding tendencies, and hepatic encephalopathy. Major causes include chronic alcohol consumption, hepatitis B and C infections, non-alcoholic fatty liver disease (NAFLD), autoimmune hepatitis, biliary disorders, and genetic conditions such as hemochromatosis and Wilson's disease.

Resmetirom: Madrigal Pharmaceuticals, Inc.
Resmetirom (Rezdiffra; MGL-3196) is a liver-directed, selective thyroid hormone receptor-beta (THR-β) agonist being developed by Madrigal Pharmaceuticals for the treatment of Liver Cirrhosis associated with non-alcoholic steatohepatitis (NASH). The therapy is currently the most advanced pipeline candidate and is being evaluated in Phase III clinical development.
The ongoing MAESTRO-NASH-OUTCOMES Phase III trial is evaluating Resmetirom in patients with well-compensated NASH cirrhosis. The study is expected to enroll approximately 700 participants and is anticipated to conclude in December 2026.
Results from the Phase III MAESTRO-NAFLD-1 trial demonstrated encouraging efficacy. Patients treated with Resmetirom achieved a mean reduction of 6.7 kPa in liver stiffness measured by vibration-controlled transient elastography (VCTE), representing one of the largest reductions reported in patients with compensated MASH cirrhosis. Approximately 51% of patients achieved at least a 25% reduction in liver stiffness, a clinically meaningful outcome associated with reduced progression to end-stage liver disease. The therapy also demonstrated a favorable safety and tolerability profile.
Efinopegdutide: Merck Sharp & Dohme LLC
Efinopegdutide is a dual GLP-1/glucagon receptor agonist currently being evaluated in Phase II clinical development for Liver Cirrhosis. The therapy is administered subcutaneously and is designed to improve metabolic dysfunction and reduce liver disease progression.
GXHPC1: Gwo Xi Stem Cell Applied Technology Co., Ltd.
GXHPC1 is an investigational stem cell therapy currently being developed in Phase II clinical studies. The therapy is administered intrahepatically and aims to restore liver function through cellular regeneration and tissue repair mechanisms.
PHIN-214: PharmaIN
