What is Cold Agglutinin Disease?

Cold Agglutinin Disease (CAD) is a rare autoimmune hemolytic anemia in which the immune system mistakenly attacks red blood cells, particularly when exposed to cold temperatures. Cold-reactive autoantibodies cause red blood cells to agglutinate (clump together) and activate the classical complement pathway, resulting in hemolysis and chronic anemia.

Patients commonly experience symptoms such as fatigue, shortness of breath, acrocyanosis, jaundice, dark urine, and cold-induced circulatory disturbances. Severe complications may include hemolytic anemia, Raynaud's phenomenon, CAD-associated lymphoproliferative disorders, and late-onset malignancies.
