Immune Cell Engagers (ICEs) are engineered, antibody-based immunotherapies designed to redirect the body's own immune cells-primarily T cells, natural killer (NK) cells, and phagocytes-toward disease-associated target cells, most notably cancer cells. These multispecific constructs bind simultaneously to immune effector cell receptors (such as CD3 or CD16a) and target antigens expressed on diseased cells, forming a synthetic immunological synapse that triggers targeted cell killing independent of MHC presentation.

ICEs are mainly used in oncology, where they have shown strong clinical efficacy in hematologic malignancies such as leukaemias, lymphomas, and multiple myeloma, with expanding research into solid tumours. Beyond oncology, early-stage applications are being explored in autoimmune and inflammatory diseases by selectively modulating pathogenic immune cells.

Briefly, ICEs can be broadly classified into three categories: T-cell engagers, NK-cell engagers, and macrophage or phagocyte engagers, with bispecific antibodies currently representing the most clinically advanced and commercially viable format. The major driver of the ICEs market is their ability to deliver potent, off-the-shelf immune redirection with strong efficacy in refractory cancers, avoiding the complexity and cost of cell-based therapies such as CAR-T.
