Developmental and Epileptic Encephalopathies (DEE) - Patient

What are DEEs?
Developmental and epileptic encephalopathies (DEEs) are a group of rare, severe, early-onset epilepsies in which epileptic activity itself contributes to progressive cognitive, motor and behavioural impairment. They include syndromes such as infantile epileptic spasms (West syndrome), Lennox-Gastaut syndrome (LGS), Dravet syndrome (DS), epilepsy with myoclonic-atonic seizures (EMAS/Doose), CDKL5 deficiency and many single-gene epilepsies.
Children often present in the first months or years of life with multiple seizure types, resistant to standard antiseizure medicines, and long-term disability is common.

Global patient pool - high-level view
Incidence and prevalence
• A large population-based study estimated cumulative incidence of DEE at 169 per 100,000 children (≈1 in 590), while the broader group of epilepsy with developmental impairment (DEE + ID+E) reached 294 per 100,000 children.
• Point prevalence data from the same programme show 175 per 100,000 children with epilepsy plus developmental impairment, of which 112 per 100,000 were classified as DEEs.
• Recent work on single-gene epilepsies (many of which present as DEEs) suggests an incidence of about 1 per 2,120 live births (47.2 per 100,000) in one large national cohort.
In simple terms: DEE overall incidence is at least 1 in 600 children, and may be higher (≥1 in 2,000 live births) for the purely genetic DEE subset, making this a rare, but not ultra-rare paediatric neurology segment.

7MM diagnosed patient pool
One detailed commercial analysis estimates that the total diagnosed prevalent population of DEE in the 7MM (US, EU4 + UK, Japan) is about 235,000 patients.
Rough directional split (for client messaging):
• United States - roughly one-third of the 7MM pool
• EU4 + UK - roughly half
• Japan - remaining 10-15%
Most patients are children and adolescents, but a growing number of adults with genetic DEEs (e.g., SCN1A, CDKL5) are being recognised as survival improves.

Sub-syndrome patient pools (illustrative)
Within the DEE umbrella, several named syndromes create recognisable commercial subsegments:
• Infantile epileptic spasms syndrome (IESS/West syndrome) - cumulative incidence ≈ 58 per 100,000 children, making it one of the largest single DEE subtypes.
• Epilepsy with myoclonic-atonic seizures (EMAS/Doose) - cumulative incidence ≈ 16 per 100,000 children.
• Lennox-Gastaut syndrome (LGS)
o Accounts for about 1-2% of all epilepsy and 3-4% of childhood epilepsies.
o Patient organisations estimate ~48,000 people with LGS in the US and ~1 million worldwide.
• Dravet syndrome (DS)
o Incidence roughly 1 in 15,000-40,900 live births; prevalence ~1.5-6.5 per 100,000 people depending on study and geography.
Together, these named DEEs plus gene-defined DEEs (SCN1A+, CDKL5, STXBP1, KCNQ2, etc.) make up the core "addressable" DEE therapy population that payers, regulators, and sponsors typically model.

Market size and growth outlook
Different syndicated sources give somewhat different numbers, but they all point to a mid-single-digit CAGR globally:
• One dataset values the global DEE treatment market at USD 1.03 billion in 2022, projected to reach USD 1.53 billion by 2030 (CAGR ~4.5%).
• Another recent analysis estimates the global DEE market at USD 4.09 billion in 2024, growing to USD 8.24 billion by 2035, at a CAGR of 6.57% (2025-2035).
• A third, more aggressive forecast for DEE treatment projects USD 7.7 billion in 2025 and USD 12.6 billion by 2035, implying a 5.0% CAGR.

For practical client communication, you can safely position the market as:
• Current (2024-2025) global DEE treatment market:
~USD 3-5 billion (depending on whether only core antiseizure drugs are counted, or also broader supportive care)
• 2030-2035 outlook:
~USD 6-13 billion, with CAGR in the 5-7% range

Key growth drivers:
• Launch and uptake of syndrome-specific therapies (e.g., fenfluramine, stiripentol, clobazam add-ons in DS and LGS)
• Expansion of genetic testing, which increases properly labelled DEE cases and access to high-value therapy
• Increasing use of polytherapy and combination regimens in complex DEEs
• Early-stage pipelines in gene therapy and precision antisense/RNAi programs for monogenic DEEs

Market segmentation (high level)
By therapy class
• Conventional and broad-spectrum antiseizure medicines (ASMs)
Valproate, lamotrigine, levetiracetam, topiramate, clobazam etc. - high volume, heavily generic, still backbone of care (especially in LGS and mixed DEEs).
• Novel / high-value ASMs and add-on therapies fenfluramine, stiripentol, rufinamide, etc. - key revenue engines in DS, LGS and related DEEs.
• Emerging targeted therapies
Gene therapies, antisense oligos, disease-specific small molecules for SCN1A, CDKL5, STXBP1, KCNQ2 and other genetic DEEs - still mostly in clinical development but central to long-term upside.

By region
• North America & Europe - largest share today, driven by higher diagnosed prevalence, access to genetic testing and uptake of premium therapies.
• Asia-Pacific - fastest growth as paediatric neurology capacity, NGS panels and reimbursement frameworks improve.

How to use this in your reports
You can summarise DEE as:
• Patient pool:
o DEE incidence ≈ 1 in 600 children; ~235k diagnosed patients in 7MM; global prevalence in the several hundred thousand range, dominated by West, LGS, DS and genetic DEEs.
• Market:
o 2024-2025: roughly USD 3-5 billion
o 2035: approximately USD 6-13 billion, CAGR 5-7%
