Angioedema Overview
Angioedema is a condition characterized by sudden, localized swelling of the deeper layers of the skin and mucous membranes. It commonly affects the face, lips, tongue, throat, and extremities and can sometimes involve the gastrointestinal tract or airways, leading to life-threatening complications. Angioedema is often associated with hives (urticaria) but can also occur independently.
There are several types of angioedema, including allergic, drug-induced, hereditary, and idiopathic forms. Allergic angioedema is triggered by allergens such as food, insect stings, or medications and involves histamine release. Drug-induced angioedema, often caused by angiotensin-converting enzyme (ACE) inhibitors, occurs without an allergic reaction. Hereditary angioedema (HAE) results from a genetic deficiency or dysfunction of the C1 esterase inhibitor, leading to excessive bradykinin production. Idiopathic angioedema has no identifiable cause.
Symptoms include painless, non-itchy swelling, which can last hours to days. In severe cases, airway swelling can cause breathing difficulties, requiring emergency intervention. Diagnosis is based on clinical history, blood tests (C1 inhibitor levels for HAE), and response to treatment.
Management depends on the type and severity. Antihistamines and corticosteroids are used for allergic cases, while HAE requires C1 inhibitor replacement therapy or bradykinin-targeted drugs. Avoiding triggers is essential for long-term control.
