A study by Annane et al., titled "Current Epidemiology of Septic Shock," analyzed data from the Collège des Utilisateurs de Bases de données en Réanimation (CUB-Réa) database to determine the epidemiology of septic shock. The study found that in 22 intensive care units (ICUs), the overall incidence of septic shock was 8.2 cases per 100 ICU admissions. The annual incidence increased significantly over time, rising from 7.0 per 100 ICU admissions in 1993 to 9.7 per 100 ICU admissions in 2000.
In another study, Rhee et al. examined sepsis-associated mortality in U.S. acute care hospitals in their paper, "Prevalence, Underlying Causes, and Preventability of Sepsis-Associated Mortality." By reviewing medical records of over 500 patients who either died in the hospital or were discharged to hospice and did not return, they found that sepsis was present in 52.8% of these hospitalizations and directly caused death in 34.9% of cases. However, most deaths were attributed to severe chronic comorbidities, with only 3.7% of sepsis-associated deaths considered definitely or moderately preventable.

Septic Shock Overview
Septic shock is a critical condition that arises when a widespread infection leads to dangerously low blood pressure. It occurs in a subset of patients with sepsis, characterized by significant circulatory and cellular/metabolic abnormalities that are linked to increased mortality.
Septic shock is identified by persistent hypotension that requires vasopressors to maintain a mean arterial pressure of 65 mm Hg or higher, alongside a serum lactate level exceeding 2 mmol/L (18 mg/dL), despite adequate volume resuscitation. The progression from sepsis to severe sepsis and eventually to septic shock can be rapid, and once this transition occurs, mortality rates tend to rise. Symptoms of severe sepsis and septic shock can overlap and may include reduced urine output, acute confusion, dizziness, severe breathing difficulties, and cyanosis (bluish discoloration of the lips or extremities).
Patients with septic shock typically experience extremely low blood pressure that does not respond to fluid replacement and may exhibit abnormalities in blood flow, potentially leading to organ dysfunction. The risk of septic shock is elevated in newborns, infants, the elderly, pregnant women, and individuals with chronic diseases such as diabetes, cirrhosis, cancer, AIDS/HIV, kidney disease, and heart disease. The pathogenesis of septic shock involves a complex response of cellular activation that triggers the release of various proinflammatory mediators. This inflammatory response leads to the activation of leukocytes, endothelial cells, and the coagulation system. Key cytokines like tumor necrosis factor-alpha (TNF-α) and interleukin-1 (IL-1) are central to the pathophysiology of septic shock, and other vital mediators such as high-mobility group box 1 (HMGB1) protein also play significant roles.
Initial evaluation of a patient suspected of having sepsis involves a thorough medical history, physical examination, and clinical assessment, including the measurement of biomarkers like C-reactive protein and procalcitonin, which are crucial for early diagnosis and effective management. Confirmatory tests may include blood cultures, urine tests, wound secretions, and respiratory secretions. If the infection source is not immediately apparent, imaging studies such as X-rays, CT scans, MRI scans, and ultrasounds can help identify the infected areas.
Septic shock is a severe condition where even slight delays in treatment can significantly increase the risk of death. Treatment typically involves immediate antibiotic therapy, which should start within the first hour of diagnosis and be administered intravenously. Vasopressors are used to maintain adequate blood pressure when it remains low despite fluid resuscitation. Corticosteroids, which are anti-inflammatory drugs, may be administered if a patient's blood pressure and heart rate remain unstable after receiving fluids and vasopressors. Although corticosteroids may benefit patients with severe septic shock, further controlled studies are needed to confirm their efficacy.
Additional treatments may include insulin to control blood sugar levels and prevent hyperglycemia, large volumes of intravenous fluids to address dehydration and improve blood pressure and organ perfusion, and respiratory support if necessary. In some cases, surgery may be required to remove the source of infection, such as draining an abscess or excising infected tissue.

Septic Shock Epidemiology
A study by Hari et al. (2017) revealed that during a five-year period, sepsis-2 septic shock and sepsis-3 septic shock comprised 23.4% and 6.0% of admissions to adult general ICUs in England, respectively. The study found that sepsis-2 septic shock represented approximately three-quarters of the severe sepsis cases, with the extrapolated incidence rate rising from 69 to 79 cases per 100,000 person-years. In contrast, sepsis-3 septic shock constituted only about one-fifth of the sepsis cases, with a relatively stable incidence rate of around 19 cases per 100,000 person-years over the same period.
Additionally, both Sepsis-2 severe sepsis and Sepsis-3 sepsis together accounted for a third of all admissions to adult general ICUs in England during the study period. The extrapolated incidence rates for both conditions increased similarly, from 88 to 102 cases per 100,000 person-years.
