Invasive hemodynamics have historically been a cornerstone in the management of valvular heart disease (VHD). In the 1960s, detailed hemodynamic assessment in the catheterization laboratory was a prerequisite to establish the diagnosis of VHD, and to determine the need for corrective surgery . Advances in medical sonography in the 1980s led to a gradual replacement of invasive hemodynamics with 2-dimensional and Doppler echocardiography. However, the advent of aortic and mitral balloon valvuloplasty allowed invasive hemodynamics to retain a role, albeit limited, through the 1990s to 2000s). In recent years, the unprecedented revolution in transcatheter valvular therapies renewed the interest in invasive hemodynamics. There is a growing evidence supporting the value of invasive hemodynamics in ascertaining diagnoses, guiding transcatheter interventions, and providing useful prognostic data in patients with VHD. Hence, time has come for structural interventionalists to relearn classic concepts and develop new observations to guide therapeutic procedures. In this paper, we review the techniques and the data supporting utility of invasive hemodynamics in guiding contemporary transcatheter VHD interventions.
Direct left atrial pressure (LAP) monitoring is gaining wide acceptance as a useful tool to optimize mitral valve (MV) interventions. LAP can be obtained via a dedicated LA catheter inserted in an antegrade or retrograde fashion, or by transducing the side port of the interventional guiding sheath. Pulmonary arterial wedge pressure may be used as a surrogate to LAP when direct measurement is not feasible. Although the mean pulmonary arterial wedge pressure is similar to the mean LAP, the inherent delay and dampening of the pressure contours from the transmission through the pulmonary circulation limits the assessment of transmitral gradients or true height of the V-wave. It should be noted that routine transcatheter MV interventions are successfully guided by modern 3-dimensional echocardiography. Direct LAP measurements in these cases should be viewed as a complement and not as a substitute to high-quality echocardiographic assessment.
