Digitisation and Integration of E-Pharmacy Services with Personal Healthcare Records

Active Wellness, a web 3.0 health portal, now launches ActiveMED, a division providing drug management, fulfillment, monitoring services that are integrated end to end with the holistic healthcare services offered. This is a Comprehensive (MTM) Medication Therapy Management function that goes beyond the hospital boundaries and extends into the community, outpatient & home setting. It bridges the gap in medication management that exists in this segment. Drug therapy is the first line of defense when it comes to therapy. One in three people are on some medication. Further due to growing age of population there is the increase in the cases of chronic diseases and lifestyle disorders. These require 1-4 pills daily. Unlike infections where the treatment is short and ends in a cure; medications in chronic diseases are for the long term and in fact in some case till the end of life. This means exponential increase in medications. The compounding of age, medical factors & increasing medication complexity, at times, leads to medication errors, drug-drug interactions and toxicity. This currently accounts for 14% of the medical care costs. This if not addressed is going to go in for an exponential increase. This is further compounded by the falling compliance which in most of the cases follows the rule of 6. Here 60% of the people take long term medication for 60% of the time this to starts faltering after six months. This vicious cycle needs to be checked and corrected to prevent a spurt in costs and adverse events. To prevent adverse events there is a need of a solution that stands integrated with the PHR and that considers the past and ongoing medication to flag issues that could lead to adverse events.

ActiveMED medication management does precisely that. It is integrated with and embedded within the Personal Health Record (PHR) system that is accessible to both the consumer and the Physician. This tight integration enables safe drug prescription and improved compliance. Prescription writing within the integrated practice management system undertakes real-time checking of medications being prescribed with both the ongoing and past medication and disease history. It generates alerts that warn of interactions and toxicities. This assists physicians and helps prevent / lower the errors in medication leading to lower adverse events and medical costs. A checked and safe prescription is the start point of therapy for a patient.

ActiveMED delivers a safe prescription to the patient who has the E-commerce platform convenience to digitally convert the prescription into a drug order and submit it to the pharmacy for its fulfillment. This lowers / removes the transcription errors that may happen at the pharmacy fulfillment side.
ActiveMED's unique feature is the robust Tracking & Monitoring of medications. On receipt of the drug from the pharmacy the drug monitoring of dose intake & the time of intake gets activated. The activation starts the process real-time alerts and reminders to improve compliance. Any failure of the patient to update intake generates an alert to the next of kin which is a unique element to increase compliance. It does not stop there it monitors & tracks the drug intake and draws up a compliance report. Queries / popups ask the consumer at appropriate intervals for known and unknown side effects. Serious ones are escalated to treating physician for action.

ActiveMED above all gathers data on the efficacy and effectiveness. This gives insights to the treating team and response of drug under different disease and health conditions. This is critical pharmaco-vigilance data that facilitates drug surveillance. It addresses the issue of missed drug refills by generating refill alerts for fulfillment well in advance to prevent the dropping of a dose.
ActiveMED is integrated with the ActiveLAB gene testing to determine and deliver on the personalized precision therapy by looking at the pharmacogenetics profile of the person among other things.
