Technological breakthrough in Total Hip Replacements helps 20 year old Sai Prasad Naik lead normal life
A first of its kind Total Hip Replacement Surgery was recently performed in Goa on a 20 year old patient suffering from post traumatic avascular necrosis of his right hip. The surgery was performed at NUSI Wockhardt Hospital, Cuncolim, by Dr. Mudit Khanna, Consultant Joint Replacement Surgery, using the latest, totally un-cemented, large diameter, ceramic on ceramic, Delta motion technology.Thanks to the challenging surgery, young Sai Prasad can now lead a normal life. This surgery is yet another milestone in NUSI Wockhardt Hospital’s initiative to bring Goa -- largely known for its tourism industry -- on the national and international map for excellence in medical care.
Commenting on the case, Dr. Mudit Khanna said “Until recently, total hip replacement was not an option for the young and active as they would wear out the traditionally implanted hip components in a few years time, necessitating a more complex and demanding revision surgery in the near future. These patients were often advised to undergo hip fusion for the risk of dislocating their implanted hip if they would not follow the long list of cautions and precautions that were necessary with traditional implants.
“Hip fusion in such patients was often preferred, but only as a ‘bail-out’ lesser evil that resulted in painless and stable but a completely immobile joint. Consequent significant limitation to their mobility resulted in a huge loss to their productive years besides compromising their quality of life forever.”
Traditional hip replacement surgeries are not advised in the young as these prosthesis have only a limited life span that is inversely related to activity. Conventional implants use metal on plastic technology and the plastic component has a high tendency to wear out after a limited period of use, proving a great limiting factor.
The younger and more active the patient, the shorter is the life of the prosthesis. Once the plastic wears out, patients have to undergo a complex and demanding revision surgery. Flexibility and range of movement is yet another issue with traditional implants as they use a small diameter head which does not allow full range of movements like sitting cross legged, squatting or kneeling as these activities may cause the ‘small’ ball to pop out of the socket, a condition known as dislocation of the hip.
The novel completely un-cemented system with a large diameter head and ceramic on ceramic articulation using the delta motion technology provides a tremendous technological breakthrough in the management of these young patients. The large diameter head allows a greater uninhibited range of motion allowing activities like squatting and sitting cross legged, essentially meaning a life free from restrictions. The ceramic on ceramic interface being significantly resistant to wear offers the potential of lasting a lifetime.

Mr Sai Prasad Naik, a 20 year old college student had developed Post Traumatic Avascular Necrosis of his right hip secondary to a near fatal road traffic accident which he sustained about eight months back. At that moment he had sustained a posterior dislocation of his right hip from a rare fracture of the femoral head, compromising the blood supply to the femoral head, causing it to gradually die off from the lack of blood supply.
Although he recovered from all major injuries and several long bone fractures that he had sustained during the initial traumatic accident, the pain in his right hip worsened to a point where he found it impossible to continue with even basic activities of daily living.
Dr Mudit Khanna comments, “We were committed in our efforts to return this young boy to a life as close to normal as possible. We wanted to give him a hip that could not only potentially last his lifetime, but also one, that would allow him an uninhibited range of motion including the ability to squat and sit cross legged.
“Essentially we wanted to give him a ‘normal’ life free from restrictions and hence recommended implanting the latest ceramic-on-ceramic joint using Delta Motion Technology that had never before been used in the state of Goa.”
The surgery was performed through a minimally invasive approach from which the patient made a steady and uneventful recovery. Three months after the challenging surgery, Mr. Sai Prasad Naik is a happy young man free from pain and limp, leading a normal life free from restrictions thanks to the great technological breakthrough in the field of Total Hip Replacements in the young.
