PLYMOUTH, Michigan, 11/30/2020 – Personalized Nursing LIGHT House, Inc. (PNLH) is pleased to announce the opening of its Extended Care Program in Plymouth, Michigan. This new program will provide willing clients with a full 12 months of Peer Recovery Coach Services (individuals and groups sessions) post treatment to ensure clients successfully navigate the challenges of early recovery.
The Extended Care Program mimics, in part, various programs around the nation that provide long term follow up and support to health care professionals (typically, doctors and nurses in recovery) which often have a very high success rate. This success rate is attributed to, in part, the ongoing monitoring, feedback and tracking of persons enrolled in such programs for up to 5 years post treatment. The aim of PNLH’s Extended Care Program is to provide the persons served at PNLH one full year of support similar to the programs offered to medical professionals. This will provide the ongoing support, encouragement, linking, coordinating and accountability to the persons served for up to 1 year.
“We need to treat addiction like the chronic disease it is and that includes long term follow up and supportive services,” stated Joseph Paliwoda, CEO and Co-founder of Personalized Nursing LIGHT House. “PNLH believes, based on published research, that this new extended care program will enhance the long-term recovery rates of the clients who enroll in the program”. Mr. Paliwoda went on to state how very grateful he was to the Michigan Health Endowment Fund for their support of this program, giving PNLH the opportunity to launch the program and provide a full 12 months of peer recovery support post treatment. “I hope to demonstrate to various funding sources the efficacy of funding extended programs like the one we are piloting here” he stated.

Persons who willingly participate in this extended care program will receive contact by PNLH’s Peer Recovery Coaches and or Case Managers 2-4 times per month to sustain the relationship with the person served. Urine drug screens will be required periodically to assess the client’s status and ensure conformance to their recovery goals. Telephone support, face to face meetings, groups, and transportation to meet other treatment related goals (dentists, primary health appointments, etc.) will be provided.

"Addiction is the only disease where once you leave treatment you rarely receive follow up appointments,” stated April Meadows, PNLH’s Peer Recovery Coach Supervisor leading this new program. “That is why we created the Extended Care Program here at Personalized Nursing LIGHT House so that way we can support our clients once they have gone into the community. We make sure that they know their long-term recovery is important to us as well - I consider it an ongoing recovery check-up"

PNLH has provided clinically effective services since 1990 and offers a full continuum of care for persons suffering with substance use disorder. PNLH serves over 2,000 admissions per year and has five service locations in Plymouth, Canton, Farmington, Madison Heights and Dearborn Heights.
