Epidemiology of Diabetes Mellitus in India

Backgrounder: Globally, the number of diabetes patients has risen sharply. While in 1985, 30 million people had diabetes, the number rose to 150 million in 2000. In 2010, 285 million people (6.6% of the global population in the age group 20-79) were found to be diabetic. However, by 2030, an estimated 435 million people are expected to suffer from this disease -- 7.8% of the adult population.
Age-wise, the IDF report says, the worst affected are the 40-59-year-olds. By 2010, 132 million people in this age group are expected to suffer from diabetes. However, by 2030, this number will increase to 188 million. The report points out another interesting trend -- women are the worst affected by this disease. In 2010, one million more women than men have diabetes (143 million women as against 142 million men). The difference is expected to increase to six million by 2030 (222 million women against 216 million men).
The urban population, as expected, has a higher incidence of the disease. By 2010, the number of people with diabetes in urban areas will be 113 million compared to 78 million in rural areas. By 2030, it is expected that this discrepancy will increase to 228 million people with diabetes in urban areas and 99 million in rural communities. Globally, by 2010, four million deaths in the 20-79 age group will be due to diabetes -- 6.8% of global all-cause mortality in this age group. Majority of these deaths will be in India, China, US and Russia.
As per IDF the disease will prove costly for India, both in terms of lives lost and money wasted. In India, it will kill around 10.07 lakh people in the age group of 20-79 years every year -- the majority being women (5.81 lakh) from 2010.

The recent Survey conducted by Metropolis Healthcare

Time frame: April 2010-September 2010 (Six Months)

Sample size: 2, 28, 009 tests.

Tests done at Metropolis:
The diabetes detection tests: FBS & PPBS
The diabetes monitoring test: HbA1c

City coverage:
Western zone: Mumbai, Pune, Ahmedabad and Surat
South zone: Chennai and Kerela
North zone: Delhi

City wise break –up:
Out of the sample nos. the highest number of tests have been done in Chennai i.e.33%
The tests performed in Mumbai & Pune were 21% and 19% respectively
While in other cities accounted to less than 10% tests conducted.

Test wise break –up:
Out of the total sample tests done, 50% walk-ins appeared for FBS tests
For Post Prandial Blood Sugar tests the percentage was nearly 31%.
While only 19% attendees appeared for HbA1c tests.

Findings of the Survey

• Epidemiology of diabetes is different in different regions of India
• Mumbai city is worst affected than other cities.
• In Mumbai city, the plasma glucose abnormality was found to be highest compare to other cities.
• In all other cities the plasma glucose abnormality was found to be higher in age group of above 55 years and the next affected age group is 35-55 years. While in Mumbai the abnormality was found to be same across all the age groups which shows there is early onset of diabetes.
• Except Mumbai & Delhi, in other cities, higher percentage of males was affected than females. While in Mumbai the percentage abnormality was more in females which can be due to stress factor in females as they are burning themselves at both the ends managing family & career most of the time. In Delhi too, this ratio was found to be almost at par with that of Mumbai.
• When monitored for diabetes control, Ahmedabad was found to have the lowest diabetes control as compared to other cities eg Chennai. In Mumbai diabetes control was found to be average
• In short there is warning alarm for Mumbai as diabetes Mellitus is known as silent killer disorder as it affects vital organs like heart, kidney etc
• The recent pan India survey conducted at Metropolis on heart related diseases also showed that the age group above 30 years is more prone to cardiac disease. It is also a known fact that patients with diabetes are also at a high risk of containing heart diseases. This in turn can have long lasting effect on the health of a nation.
