Filler Type and Reversibility

Dr. Sayah typically recommends beginning with hyaluronic acid fillers, such as those in the Juvederm or Restylane families, because they can be dissolved with hyaluronidase if adjustment is necessary.

Thicker, more structural formulations are often used in the nose compared to softer fillers placed in lips or under-eye areas.

In certain cases, longer-lasting fillers may be considered - but only after careful evaluation of anatomy, candidacy, and long-term goals.

Rare but Serious Risk: Vascular Occlusion and Vision Loss

One of the most critical portions of the discussion addresses vascular occlusion - a rare complication in which filler enters a blood vessel and may compromise blood flow, potentially affecting vision.

"There is a very rare risk that filler can enter a blood vessel and cause partial or complete blindness," Dr. Sayah states. "It is uncommon. But when it occurs, it is serious."

To reduce risk, he describes several technique considerations, including:

• Injecting while withdrawing the needle
• Using blunt-tip cannulas when appropriate
• Closely monitoring patients immediately after injection
• Maintaining hyaluronidase on site for emergency reversal
