"From the age of 40, on average only about one in five blastocysts is genetically intact, i.e., chromosomally healthy (euploid)," explains Dr. Katharina Spies, reproductive medicine specialist and medical director of the Vida Fertility Clinic, who has been working with patients in this age group on a daily basis for almost 15 years.

In practice, this means that even embryos with good morphological quality often have chromosomal changes that make implantation or healthy development impossible.

What is PGT-A and why is it so important after 40?

PGT-A (preimplantation genetic testing for chromosome analysis) is a genetic procedure in which embryos are examined before transfer. The aim is to select only embryos with a complete and correct set of chromosomes. This method is particularly crucial for women over 40. Without PGT-A, embryos that have no biological chance of resulting in pregnancy are often transferred.

"Without this analysis, many women experience several unsuccessful transfers in a row. This not only costs time and money, but above all emotional energy," emphasizes Dr. Spies. In Germany for instance, PGT is only permitted in very limited exceptional cases. In Spain, on the other hand, PGT-A has been an integral part of modern fertility treatment for years.

Egg donation: an established option with long experience

If the quality of your own eggs is no longer sufficient, egg donation may be another realistic option. In Germany, it is not provided for by law, whereas the donation of male gametes is permitted. Spain, on the other hand, has almost 40 years of clinical experience with egg donation. This treatment is established and accepted both medically and socially. "Today, there are clinics that speak fluently many languages, understand the legal differences, and can present realistic options. Waiting doesn't help anyone at this age."
