Definition:
Tuberculosis (TB) treatment drugs refer to a range of pharmaceutical agents used to combat the infection caused by the bacterium Mycobacterium tuberculosis, which primarily affects the lungs but can also impact other parts of the body. TB treatment typically involves a combination of antibiotics, as the bacteria are known to develop resistance to drugs when treated with single-agent therapy. The most commonly used drugs in TB treatment include Isoniazid, Rifampicin, Ethambutol, and Pyrazinamide, which are usually prescribed together in a regimen referred to as the "first-line" treatment for TB. This regimen is designed to attack the bacteria from different angles, ensuring effective eradication and reducing the risk of resistance. Isoniazid works by inhibiting the synthesis of mycolic acid, a crucial component of the bacterial cell wall, while Rifampicin interferes with bacterial RNA synthesis. Ethambutol blocks the formation of the bacterial cell wall, and Pyrazinamide helps to lower the pH of the environment inside the cell, making it more hostile to the bacteria. These drugs are typically administered for a period of six months, although treatment duration can be longer in cases of drug-resistant TB or extrapulmonary TB (when TB affects organs other than the lungs). For drug-resistant TB, more complex regimens involving second-line drugs are necessary. Second-line drugs may include Fluoroquinolones, such as Levofloxacin or Moxifloxacin, and injectable antibiotics like Amikacin or Capreomycin. In cases of extensively drug-resistant tuberculosis (XDR-TB), which is resistant to both first-line and second-line drugs, treatment options become significantly limited, and newer drugs such as Bedaquiline and Delamanid are used in combination with other drugs to improve efficacy. These newer drugs are crucial in the fight against multidrug-resistant (MDR) and XDR-TB, as they offer a lifeline in situations where traditional drugs fail.
