The exact cause of sporadic ALS remains unclear, with several potential mechanisms proposed, including abnormal RNA processing, SOD1 mutations leading to free radical damage, inflammation, and excessive glutamate. Familial ALS is more rare and often involves genetic mutations, such as C9ORF72 gene repeat expansions and SOD1 mutations. Both genetic forms are inherited in an autosomal dominant pattern, contributing to the disease's complex etiology.

Respiratory issues are a major concern in ALS patients due to weakening of the muscles involved in breathing. Noninvasive ventilation is often recommended for symptoms like difficulty breathing or low forced vital capacity (FVC). If necessary, invasive ventilation may be considered. Swallowing difficulties (dysphagia) are common and managed with diet changes, and enteral feeding via a G-tube may be needed to address weight loss.

Complications of ALS include respiratory decline, dysphagia, speech and muscle problems, malnutrition, spasms, fatigue, and issues with salivation and mucus production. Additionally, some medications used to manage ALS symptoms can cause side effects, such as gastrointestinal upset, liver issues, or headaches.
