Candesartan cilexetil is an angiotensin-receptor blocker (ARB) prodrug that is rapidly converted to candesartan, the active metabolite, in the gastrointestinal system after absorption. By antagonizing the hypertensive effects of angiotensin II throughthe RAAS, candesartan lowers blood pressure. The RAAS system regulates hemodynamic, water, and electrolyte balance in the body. Renin is released by granular cells of the juxtaglomerular apparatus in the kidneys after sympathetic stimulation or when renal blood pressure or blood flow is lowered. Renin converts circulating angiotensinogen to angiotensin I, which is then converted to angiotensin II by angiotensin-converting enzyme (ACE).

Angiotensin II raises blood pressure by modifying the cardiovascular structure and raising total peripheral resistance. It also increases sodium and water reabsorption in the kidneys via aldosterone release. The absolute bioavailability of candesartan was calculated to be 15% after administration of the candesartan cilexetil prodrug. The bioavailability of candesartan from candesartan cilexetil is unaffected by high-fat foods.Type-1 angiotensin II receptor (AT1) and type-2 angiotensin II receptor (AT2) are the two receptors that angiotensin II binds to (AT2). Angiotensin II's vasoconstrictive and aldosterone-secreting effects are mediated by AT1, a G-protein coupled receptor (GPCR). AT2 antagonises AT1-mediated effects and directly affects long-term blood pressure control by inducing vasorelaxation and increasing urinary sodium excretion.
